Complex Care Case Manager (LCSW/LICSW/LMHC) - Summary- Provide intensive case management and care coordination for high ED utilizers within a Federally Qualified Health Center (FQHC) setting.

- Master’s degree in Social Work, Mental Health Counseling, or a related Human Services field.
- Active, unrestricted license as LCSW, LICSW, or LMHC.

- Certification in Case Management preferred but not required.
