Community Paramedic (CP-C, Paramedic) - Summary

Duties & Responsibilities
- Conduct thorough in-home patient assessments and clinical evaluations to determine health status and identify medical needs
- Obtain vital signs, perform EKGs, and administer point-of-care testing as indicated
- Support and facilitate telemedicine and virtual provider visits to enhance patient access to comprehensive care
- Administer intramuscular (IM) and intravenous (IV) medications and perform basic clinical procedures within the paramedic scope of practice
- Develop and coordinate care plans with healthcare providers, ensuring seamless communication and effective care delivery
- Identify and address barriers impacting patient health and safety, including social determinants of health and environmental factors
- Coordinate with EMS, hospitals, and community resources to ensure continuity of care and appropriate referrals
- Maintain accurate, timely documentation of all patient interactions and interventions
- Participate in ongoing training and professional development

Qualifications & Requirements
- Current, unrestricted Paramedic license
- BLS (Basic Life Support) and ALS (Advanced Life Support) certifications required
- Valid driver's license and reliable transportation
- 5+ years of experience in EMS, Community Paramedicine, or Mobile Integrated Health preferred
- Experience working with geriatric populations, telemedicine, or home-based care strongly preferred
- CP-C (Community Paramedic Certification) required within 18 months of hire
