Community Health Worker (Mandarin/Cantonese Bilingual) CHW - Summary
- Deliver culturally competent health navigation and social support to members, focusing on those with limited English proficiency and significant social or health barriers.
- Utilize motivational interviewing to build trust, set goals, and encourage positive behavioral changes in Mandarin or Cantonese, as required by the member.
- Serve as a resource connector, empowering members to access community-based services related to food, housing, behavioral health, transportation, and more.
- Work closely with interdisciplinary teams to ensure continuity and quality of care, maintaining meticulous documentation of all member interactions.
Duties & Responsibilities
- Apply motivational interviewing techniques to establish rapport and support members in achieving goals related to health, nutrition, housing, and overall well-being.
- Conduct proactive outreach via phone, text, or video to assess member needs and provide tailored one-on-one support in Mandarin or Cantonese.
- Guide members to appropriate local programs and resources, including food assistance, housing support, behavioral health services, and transportation solutions.
- Collaborate with internal teams such as dietitians, member support staff, and clinical partners to ensure seamless transitions and comprehensive support for each member.
- Maintain accurate and timely records of all member interactions and care planning in internal systems.
- Conduct ongoing follow-ups to monitor member progress, address obstacles, celebrate achievements, and adjust care plans as needed.
- Deliver empathetic, trauma-informed, and culturally respectful support throughout all interactions.
Salary & Benefits
- This is a contract position, with compensation determined based on market data and individual qualifications.
- Contracted roles do not include benefits; full-time positions may offer competitive compensation, PTO, paid holidays, health, dental, vision, and other benefits as outlined in future opportunities.
- Work equipment such as a computer or related accessories is not provided for contract roles.

Qualifications & Requirements
- Minimum of 2 years' experience in care navigation, social work, health coaching, behavioral health, or similar support roles.
- Fluency in Mandarin or Cantonese is required to deliver tailored support to linguistically diverse members.
- Proficiency in motivational interviewing or comparable methods for supporting individuals through change.
- Strong understanding of public benefit systems such as Medicaid, SNAP, public housing, or behavioral health services.
- Demonstrated experience supporting individuals from low-income backgrounds or managing chronic conditions.
- Ability to work independently in a remote setting, utilizing digital platforms (CRM, EHR) for documentation and time management.
- Occasional in-person travel may be required for training or team activities.
- Community Health Worker certification is an advantage but not mandatory.
