The Community Health Worker (CHW) serves as a trusted partner to our members, supporting them remotely through proactive outreach and motivational interviewing. This position is designed for individuals who are passionate about helping others, have experience in care coordination or social support roles, and excel at building rapport across diverse populations. You will have the opportunity to deliver trauma-informed, culturally respectful guidance while working independently in a dynamic, technology-enabled environment.
Community Health Worker (CHW) – Summary
- Contract role supporting Tangelo members in accessing community resources and achieving health-related goals.
- Utilizes motivational interviewing, proactive outreach, and strong documentation skills to provide individualized support.
- Works remotely, with occasional in-person activities as required for training or special initiatives.
- Acts as a bridge connecting individuals to programs focused on nutrition, housing, behavioral health, transportation, and more.
Duties & Responsibilities
- Apply motivational interviewing techniques to encourage members to make progress towards their health and life goals, including nutrition, housing, and overall well-being.
- Conduct outreach to members via phone, text, or video to assess needs and deliver one-on-one guidance.
- Connect members to appropriate local programs, services, and resources, such as food programs, housing assistance, and behavioral health support.
- Collaborate with internal teams, such as dietitians, member support, and clinical partners, to ensure seamless care transitions and coordinated support.
- Maintain accurate records of all member interactions and care planning activities within internal systems.
- Perform consistent follow-ups to monitor member progress, help overcome barriers, and celebrate achievements.
- Deliver support that is empathetic, trauma-informed, and culturally sensitive at every stage of engagement.
Salary & Benefits
- Compensation is competitive and based on market data, experience, and skills. Individual compensation will be discussed during the interview process for transparency and fairness.
- Contract positions do not include benefits during the contract term. Equipment such as computers, mouse, or peripherals are not provided for this role.

Qualifications & Requirements
- Minimum of 2 years’ experience in care navigation, social work, health coaching, behavioral health, or similar support roles.
- Certification as a Community Health Worker (CHW) is preferred but not required.
- Proficiency with motivational interviewing or similar change-guiding methodologies.
- Knowledge of systems and resources such as Medicaid, SNAP, public housing, and behavioral health services.
- Experience supporting individuals from low-income backgrounds or those managing chronic health conditions.
- Demonstrated ability to work independently in a remote environment, utilizing digital platforms like CRM or EHR for documentation and case management.
- Willingness to participate in occasional in-person meetings or training as needed.
