We are currently seeking a committed Community Health Navigator (Mandarin/Cantonese) to join our team. As a Community Health Navigator (Mandarin/Cantonese), you will work directly with Tangelo members to guide them in navigating community resources, setting personal health goals, and overcoming barriers to care. Your fluency in Mandarin or Cantonese and expertise in motivational interviewing are essential for providing culturally responsive and trauma-informed support. By empowering members and facilitating connections to vital services, you will play a crucial role in promoting health equity and improving community outcomes.
This contract position offers a flexible, remote-first work environment, with a 35-hour workweek. Community Health Navigator (Mandarin/Cantonese) - Summary
- Support Tangelo members in overcoming social, behavioral, and environmental barriers to health and well-being
- Utilize Mandarin or Cantonese language skills to provide personalized support
- Promote access to nutritious food and health resources through culturally responsive care
Duties & Responsibilities
- Use motivational interviewing techniques to build trust, empower members, and help them achieve health, housing, nutrition, and personal goals
- Conduct structured outreach through phone, text, or video to assess needs and offer tailored guidance
- Identify and refer members to appropriate local resources and support programs such as food assistance, housing support, transportation, and behavioral health services
- Collaborate with internal teams, including dietitians, member support, and clinical partners, to ensure seamless care coordination and continuity
- Accurately document member interactions and care plans using internal digital systems
- Maintain consistent follow-up with members to monitor progress, address challenges, and celebrate achievements
- Provide trauma-informed, culturally sensitive, and empathetic care

Qualifications & Requirements
- 2+ years of experience in care navigation, social work, health coaching, behavioral health, or a related field
- Fluency in Mandarin or Cantonese is required
- Skilled in motivational interviewing or similar supportive communication approaches
- Familiarity with Medicaid, SNAP, public housing, or behavioral health services
- Experience working with low-income individuals or those with chronic health conditions
- Comfortable working remotely and using digital tools such as CRM or EHR platforms
- Willingness to participate in occasional in-person training as needed
