As a Clinical Pharmacist (PharmD) with Decypher, you will be responsible for dispensing medications, reviewing patient profiles to ensure accuracy and safety, counseling patients on proper medication use, and collaborating with healthcare providers to resolve any medication-related discrepancies. You will oversee the compounding of medications, manage pharmacy inventory, and ensure quality control and regulatory compliance, including the secure handling of controlled substances. The position requires adherence to high standards of care and the ability to function effectively in a fast-paced clinical environment.

- Key member of the healthcare team responsible for comprehensive pharmaceutical care, including medication dispensing, patient counseling, and provider collaboration.
- Ensures quality control, regulatory compliance, and effective inventory management in a clinical pharmacy setting.
- Supports Decypher's mission to provide high-quality care for veterans, service members, and their families, in partnership with the Defense Health Agency.
Duties & Responsibilities
- Perform a full range of clinical pharmacist procedures and pharmaceutical services in accordance with facility capabilities.
- Dispense medications accurately and ensure proper review of patient profiles for safety, accuracy, and potential interactions.
- Counsel patients on appropriate medication use, side effects, and therapy expectations to optimize treatment outcomes.
- Collaborate with healthcare providers to resolve discrepancies related to prescription directions, dosages, medication interactions, therapeutic duplications, allergy information, age-specific dosages, and other relevant patient data.
- Compound medications as required, maintaining precision and adherence to safety protocols.
- Supervise and ensure the accuracy of pharmaceutical products prepared or processed by technical staff.
- Oversee inventory control of all pharmaceuticals, with special attention to scheduled substances and pharmacy security.
- Maintain accurate records and security measures for all scheduled controlled substances in compliance with applicable regulations.

Qualifications & Requirements
- Doctor of Pharmacy (PharmD) degree from an accredited institution (ACPE-accredited program required).
- Current, full, active, and unrestricted license to practice as a pharmacist.
- Basic Life Support (BLS) certification required.
- At least one year of professional pharmacy experience following graduation.
- U.S. Citizenship required for consideration.

Other Relevant Information
- Work schedule: Full-time, Monday to Friday, with 8-hour shifts between 7:00am and 5:00pm. Flexibility in scheduling is possible with prior coordination.
