Clinical Pharmacist (PharmD) - Summary

Duties & Responsibilities
- Lead and expand clinical pharmacy services, including chronic disease management for conditions such as diabetes, hypertension, and asthma
- Conduct comprehensive medication reviews (CMRs) and medication therapy management (MTM)
- Collaborate with medical providers to resolve medication-related problems and develop patient-specific care plans
- Participate in interdisciplinary care teams and population health initiatives, including quality improvement and value-based care programs
- Assist with transitions of care, medication reconciliation, patient education, and efforts to reduce hospital readmissions
- Provide patient counseling on disease states, medication adherence, potential side effects, and lifestyle modifications
- Administer immunizations and prescribe contraception in accordance with collaborative agreements and state protocols
- Oversee prescription processing and dispensing, ensuring compliance with all legal and regulatory requirements
- Serve as a subject matter expert on drug information, formulary management, and clinical protocols
- Support 340B program oversight, compliance, and optimization strategies
- Precept Pharm.D. candidates and mentor pharmacy interns and residents
- Participate in policy and protocol development focused on clinical services and patient safety
- Identify opportunities for innovation and improvement in pharmacy services
- Act as a clinical resource for pharmacy staff and other departments
- Participate in staff education, interdisciplinary rounds, and performance improvement initiatives
- Maintain commitment to continuing education, clinical training, and professional certification

Qualifications & Requirements
- Licensed Pharmacist (or eligibility) as required in the relevant state, with licensure to be obtained within 45 days of employment
- Demonstrated clinical pharmacy background; completion of PGY1 residency or equivalent experience preferred
- Certification in immunization and hormonal contraception (required within 6 months of hire)
- CPR (BLS) certification (required within 6 months of hire)
- Ability to work independently and as part of a team-based care environment
- Strong communication, organizational, and problem-solving skills
- Experience serving underserved populations and participating in value-based care is a plus
- Physical ability to lift or move up to 25 pounds and perform standard pharmacy-related tasks
- Adequate hearing, vision, and speech to fulfill job duties effectively
