As a Clinical Pharmacist, you will work on-site and collaborate with clinical staff, including physicians, nurses, dietitians, and other healthcare providers, to optimize medication regimens and support comprehensive patient care. The Clinical Pharmacist position involves maintaining a comprehensive scope of practice, including direct patient care, medication therapy management, and continuous review of electronic medical records to monitor drug therapy appropriateness and identify adverse drug reactions. You will be responsible for assessing patients’ responses to drug therapy, planning individualized treatment regimens, and ensuring that all medication-related activities align with current clinical guidelines and best practices. In addition, you will provide education to patients and their families regarding medication use, adherence strategies, and lifestyle modifications to promote overall wellness and improve therapeutic outcomes.
This role requires strong analytical, communication, and organizational skills, as well as a commitment to lifelong learning and professional development. The ideal candidate will possess a Doctor of Pharmacy (Pharm.D.) degree, active licensure, relevant certifications, and experience in clinical pharmacy practice. Clinical Pharmacist - Summary
- Provide direct patient care and comprehensive pharmaceutical services as a Clinical Pharmacist.
- Collaborate with an interdisciplinary healthcare team to ensure optimal medication management, with a focus on pain management and chronic disease states.
- Maintain a defined scope of practice and engage in continuous professional development.
Duties & Responsibilities
- Assess patients' responses to medication therapy and plan drug therapy based on physician-established diagnoses.
- Order and evaluate laboratory tests to monitor the effectiveness and safety of drug therapy.
- Initiate, modify, or discontinue medications for chronic conditions in collaboration with medical staff.
- Evaluate medical and medication histories for drug-related issues and adjust therapy accordingly.
- Consult with other healthcare providers about patient treatment options.
- Ensure the accuracy of all pharmaceutical products prepared or processed by technical staff.
- Provide medication-related training and education to clinical staff on new and existing therapies.
- Conduct clinical interviews with patients and families to design personalized pharmacotherapeutic treatment plans.
- Educate patients and caregivers on medication use, adherence, and lifestyle modifications to promote wellness.
- Document and communicate clinical findings, recommendations, and patient agreements as appropriate.
- Participate in telementoring and ongoing training programs to maintain clinical excellence.

Qualifications & Requirements
- Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited college or university, or FPGEC certificate with active U.S. licensure.
- Full, active, and unrestricted license to practice pharmacy in a U.S. state, territory, or district.
- Basic Life Support (BLS) certification.
- Completion of a clinical pharmacy internship, residency, or fellowship accredited by the American Society of Health-System Pharmacists (ASHP) or American College of Clinical Pharmacy (ACCP).
- Optional: Board of Pharmacy Specialties certification, completion of a relevant postgraduate degree, or experience in medication therapy management.
- U.S. Citizenship required.
