- Perform comprehensive health risk assessments for eligible patients.

- Document all patient encounters and services rendered in the EMR accurately and efficiently.
- Collaborate with physicians and multidisciplinary care teams to ensure optimal patient outcomes.
- Assist with rooming patients and obtaining vital signs as needed for provider visits.
- Maintain patient confidentiality and adhere to HIPAA and organizational privacy policies.

Qualifications & Requirements
