Care Navigator (CHW) – Contract Position
Tangelo is committed to improving access to nutritious, healthy foods for individuals and families in need, particularly those facing chronic conditions or economic challenges. Our innovative platform partners with various organizations to provide subsidized, medically tailored meals and grocery delivery services. We are currently seeking a dedicated Care Navigator (CHW) to join our team and support members in need of social, behavioral, or environmental resource navigation to improve their health and wellbeing. This role is a contract opportunity designed for individuals passionate about making a meaningful difference in the lives of others.
As a Care Navigator (CHW), you will use motivational interviewing and empathetic communication to help members set achievable goals, connect with community resources, and gain confidence in taking positive steps toward improved health outcomes. The contract nature of this position offers meaningful work and exposure to a mission-driven organization, while providing flexibility for professionals eager to contribute to community health efforts.
In this role, you will conduct structured outreach through phone, text, or video, assessing member needs and providing tailored support and guidance. You will collaborate closely with internal teams, such as Dietitians, Member Support, and Clinical Partners, to ensure seamless coordination and continuity of care. Your ability to document interactions and care plans efficiently and accurately will be essential to ensuring high-quality service and ongoing support for members.
Care Navigators (CHW) are tasked with identifying and referring members to appropriate local resources, including food assistance, housing support, transportation options, and behavioral health services. You will follow up consistently with members to monitor progress, address potential barriers, and celebrate achievements along the way. Your care will be trauma-informed, culturally responsive, and rooted in respect for each individual’s unique background and experience.
This position is ideal for professionals with experience in care navigation, social work, health coaching, behavioral health, or related support fields. Familiarity with systems like Medicaid, SNAP, public housing, or behavioral health services is highly valued, as is comfort with remote work and digital documentation platforms such as CRM or EHR systems. While the role is primarily remote, occasional travel for in-person training may be required.

- 2+ years of experience in care navigation, social work, health coaching, behavioral health, or related field
- Community Health Worker (CHW) certification is a plus
- Skilled in motivational interviewing or similar support approaches
- Familiarity with Medicaid, SNAP, public housing, or behavioral health systems
- Experience working with low-income individuals or those with chronic conditions
- Ability to work remotely and proficiency with digital tools such as CRM or EHR
- Willingness to attend occasional in-person training sessions
- Empathetic communicator with strong interpersonal skills
- Organized, detail-oriented, and comfortable with digital documentation
