Cardiothoracic Nurse Practitioner (NP, APRN) - Summary

Duties & Responsibilities
- Round on cardiothoracic surgical patients in step-down and general hospital units, ensuring continuity and quality of care outside the ICU.
- Conduct comprehensive patient assessments, formulate diagnoses, and develop evidence-based treatment plans in collaboration with physicians and the healthcare team.
- Implement and manage care for patients requiring advanced therapies such as mechanical circulatory and ventilatory support.
- Monitor patient progress, adjust care plans as needed, and communicate effectively with patients, families, and the healthcare team.
- Document all patient encounters, clinical findings, and interventions thoroughly and accurately in the electronic health record system.
- Provide education to patients and families about cardiac conditions, surgical procedures, recovery plans, and health maintenance strategies.
- Participate in multidisciplinary rounds, case discussions, and quality improvement initiatives to enhance patient outcomes and safety.
- Support transition-of-care planning, including discharge preparation and coordination with outpatient providers as appropriate.

Qualifications & Requirements
- Current and valid Nurse Practitioner (NP, APRN) license.
- Demonstrated experience in inpatient cardiothoracic care, preferably in a hospital setting.
- Certification as an Acute Care Nurse Practitioner or related specialty is preferred.
