RN Case Manager - Summary- Coordinate and oversee patient care plans across the healthcare continuum
- Work collaboratively with patients, families, and interdisciplinary teams
- Advocate for patient needs and optimal health outcomes
- Ensure compliance with relevant clinical protocols and regulatory standards
- Utilize clinical judgement to assess, plan, and evaluate patient care
- Conduct comprehensive assessments of patient needs
- Develop, implement, and monitor individualized care plans
- Facilitate communication and collaboration among healthcare providers, patients, and families
- Advocate for patient access to necessary healthcare services and resources
- Ensure accurate documentation and timely reporting in accordance with facility policies
- Monitor and evaluate patient progress, adjusting care plans as necessary
