In this full-time position, the RN Case Manager will engage in a variety of essential functions, including comprehensive patient assessments, developing and implementing individualized treatment plans, and collaborating with healthcare providers to deliver seamless care transitions. - Conduct comprehensive case management services including patient assessment, care planning, coordination, implementation, monitoring, and evaluation.
- Develop and implement local case management strategies utilizing inpatient, outpatient, onsite, and telephonic approaches.
- Create and oversee treatment plans that include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions.
- Coordinate patient care and ensure smooth transitions post-discharge in ambulatory and community healthcare settings.
- Monitor patient progress, reassess care plans as needed, and communicate effectively with patients, families, and healthcare professionals.
- Maintain accurate and complete medical records in compliance with regulatory guidelines.

- Associate Degree in Nursing required; Bachelor of Science in Nursing preferred.
- Active, unrestricted Registered Nurse (RN) license.
- Basic Life Support (BLS) certification required.
- One or more of the following certifications: Certified Case Manager (CCM), Certified Disability Management Specialist (CDMS), Certified Rehabilitation Registered Nurse (CRRN), Certified Occupational Health Nurse (COHN), Certified Occupational Health Nurse-Specialist (COHN-S), Advanced Certification in Continuity of Care (ACCC), Certified Rehabilitation Counselor (CRC), Nurse Case Manager (RN-NCM), or Care Manager Certified (CMC).
- Minimum of four (4) years of experience in discharge planning or case management for Associate Degree holders.
- Minimum of two (2) years of full-time experience in discharge planning or case management for Bachelor’s degree graduates. Full-time employment in a nursing field within the last 36 months is required.
- U.S. Citizenship required.
- Demonstrates strong clinical judgment and critical thinking skills.
- Has excellent communication and interpersonal abilities to collaborate with healthcare teams and support patients and families.
- Exhibits attention to detail and high standards of accuracy in documentation and patient care planning.
- Shows commitment to ongoing professional development and continuous learning in case management best practices.
- Displays flexibility and adaptability in dynamic healthcare environments.
- Work environment is primarily onsite, with standard office hours.
- Physical requirements include the ability to sit or stand for extended periods, review medical records, operate standard office equipment, and occasionally lift up to 15 pounds.
