Nurse Case Manager RN Job Description
The Nurse Case Manager RN is responsible for conducting comprehensive patient assessments and managing healthcare needs across the care continuum. This full-time position focuses on enhancing individual health outcomes and promoting self-management skills by collaborating with a multidisciplinary team that includes behavioral health professionals and community health workers. The Nurse Case Manager RN works both in-office and within hospital settings, performing face-to-face visits with members who require in-person assessments and interventions. By coordinating and facilitating services that align with members' benefit structures and leveraging community resources, the Nurse Case Manager RN ensures that each patient receives appropriate, cost-effective, and high-quality care.
In this role, the Nurse Case Manager RN partners closely with an interdisciplinary care team to develop and implement individualized care plans in collaboration with each patient. The focus remains on supporting quality outcomes in the ambulatory setting, aiming to improve health status and patient satisfaction. Embedded within provider offices, the Nurse Case Manager RN works alongside primary care providers, office staff, care management personnel, and community partners to manage all aspects of member care. This collaborative approach ensures seamless transitions of care and optimal patient experiences.
The Nurse Case Manager RN supports the organization's mission, vision, and values by adhering to established compliance protocols and organizational policies and procedures. The role requires effective communication, strong organizational skills, and the ability to work independently while managing a dynamic caseload. The Nurse Case Manager RN is expected to demonstrate cultural competence and sensitivity while working with a diverse member population, including those with complex medical, behavioral, and social needs.
Responsibilities
- Conduct comprehensive patient assessments and manage care coordination throughout the care continuum.
- Perform in-person visits in office or hospital settings as required for patient assessments and interventions.
- Develop individualized care plans in collaboration with patients and the interdisciplinary care team, with a focus on ambulatory care settings.
- Coordinate services that meet members' needs according to benefit structures and available community resources.
- Work collaboratively with providers, office staff, care managers, and community partners to manage member care and ensure positive outcomes.
- Support organizational mission, vision, and values while following compliance and policy protocols.
- Facilitate communication between patients, providers, and other members of the care team.
- Document care coordination activities, patient progress, and outcomes accurately and efficiently.
- Utilize telephonic interviewing skills and provide support to a diverse and multicultural patient population.
- Assess patients' activities of daily living and functional independence to inform care planning.
Qualifications
- Minimum of 2 years of clinical experience as a Registered Nurse (RN) working with patients across the lifespan.
- Experience working with individuals on MassHealth coverage and those with complex medical, behavioral, or social needs is preferred.
- Familiarity with the hospitalization process and post-discharge care needs required.
- Ability to communicate and collaborate effectively with primary care providers, community, and ACO partners.
- Experience with telephonic interviewing and working with a diverse, potentially non-English speaking population required.
- Home Health Care or community social service agency experience preferred.
- Experience with case management, care coordination, and self-scheduling of caseload preferred.
- Demonstrated ability to work independently with minimal supervision.
Education and Certifications
- Registered Nurse (RN) licensure required.
- Graduation from an NLN-accredited nursing program required.
- Bachelor's or advanced degree in nursing or a healthcare-related field preferred.
- Current CPR certification required (must be renewed every two years).
- Active and valid driver's license required.
- RN license as a professional nurse within Massachusetts required.
