Primary Care Physician (PCP) – Atlanta, GA – Full-time
Join a mission-driven, physician-led primary care organization serving the Atlanta, GA community as a Primary Care Physician (PCP). This full-time role offers an opportunity to make a meaningful impact on the health and well-being of senior patients, supporting a vision of delivering affordable, VIP-level healthcare. The organization is recognized for its commitment to transforming care for seniors with complex health needs through value-based care and patient-centered services.
Responsibilities
- Provide comprehensive outpatient primary care to a panel of up to 450 senior patients, focusing on those with multiple chronic conditions.
- Deliver care in a 100% outpatient setting with no night or weekend shifts, supporting an optimal work/life balance.
- Manage an average of 18 patients per day, ensuring personalized and thorough medical attention.
- Collaborate closely with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to optimize patient outcomes.
- Utilize value-based care strategies to improve health outcomes, patient satisfaction, and cost-effectiveness.
- Participate in administrative duties with protected time built into the schedule.
- Work Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m., and the last patient scheduled at 4:15 p.m.
- Engage in continuous collaboration with in-house consulting specialists in fields such as Cardiology and Podiatry, as well as holistic health providers offering services like acupuncture, Tai Chi, yoga, and nutrition counseling.
Qualifications
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, patient-focused care within a value-based model.
- Strong communication and teamwork skills, with a dedication to improving care for seniors.
