Primary Care Physician (PCP) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our mission-driven, physician-led primary care organization in Atlanta, GA. As a Primary Care Physician, you will play a critical role in improving the health and well-being of a senior patient population, working in a collaborative environment that prioritizes both patient care and professional development. Responsibilities
- Provide comprehensive, primary care services to a patient panel of up to 450 individuals, with a focus on seniors managing multiple chronic diseases.
- Conduct thorough patient assessments, develop individualized care plans, and deliver value-based care in a 100% outpatient setting.
- See approximately 18 patients per day, ensuring each patient receives the time and attention needed for optimal care.
- Work collaboratively with a dedicated support team, including a Medical Assistant, Scheduler, and Referral Specialist, to coordinate patient care and administrative tasks.
- Participate in clinic operations, including administrative time built into your schedule to ensure efficient patient management.
- Maintain regular clinic hours from Monday to Friday, 7:30 AM to 5:00 PM, with no nights or weekends required, supporting a healthy work/life balance.
- Engage in partnership opportunities, with the potential to become a partner within 12-18 months based on performance and organizational fit.
- Utilize in-house consulting specialists, such as cardiology and podiatry, to coordinate comprehensive care for complex patient needs.
- Promote holistic health by supporting services like acupuncture, Tai Chi, yoga, and nutrition counseling offered within the clinic.
- Ensure patients have access to necessary care by leveraging door-to-door transportation services provided by the clinic.
Qualifications
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in either Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing patient-centered, value-based care to a senior population with complex health needs.
- Strong communication, teamwork, and organizational skills to effectively collaborate with multidisciplinary care teams.
