Primary Care Physician (PCP) Overview
Our organization is seeking a dedicated and compassionate Primary Care Physician (PCP) to join our mission-driven team in Philadelphia, PA. This is a full-time opportunity for a board-certified or board-eligible physician to provide comprehensive outpatient care to an underserved senior population. Responsibilities
- Provide comprehensive primary care services to a panel of senior patients, focusing on preventive care, chronic disease management, and overall wellness.
- Manage a maximum patient panel of approximately 450 individuals, allowing for personalized and attentive care.
- Conduct outpatient visits, averaging 18 patients per day within clinic hours of 7:30 a.m. to 5:00 p.m., Monday through Friday, with the last patient scheduled at 4:15 p.m.
- Participate in a value-based care model, emphasizing quality outcomes and patient satisfaction.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to optimize patient care and ensure seamless coordination of services.
- Utilize administrative time included in the schedule to complete documentation and coordinate care.
- Refer patients as needed to in-house consulting specialists such as cardiology and podiatry, and support patients' access to holistic health services including acupuncture, tai chi, yoga, and nutrition counseling.
- Ensure transportation needs are met for eligible patients through the organization’s complimentary service.
Qualifications
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, compassionate care to seniors, particularly those with multiple chronic conditions.
- Spanish language proficiency is preferred, but not required.
- Strong interpersonal and communication skills, with the ability to work effectively within an interdisciplinary team environment.
