We are seeking a dedicated and compassionate Primary Care Physician (PCP) for a full-time outpatient position in Tucson, AZ. Responsibilities
- Provide comprehensive primary care services for an adult and senior patient population, primarily managing patients with complex chronic illnesses such as heart disease, diabetes, and hypertension.
- Deliver care in an outpatient setting, focusing on preventive health, chronic disease management, and patient education.
- Practice within a value-based care model, supporting initiatives that enhance patient outcomes and satisfaction.
- Maintain continuity of care by cultivating relationships with patients and their families, ensuring a personalized approach to healthcare delivery.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership, to optimize patient care.
- Manage a panel size of up to 650 patients, averaging 10-12 patients per day, allowing for focused attention and comprehensive visits.
- Participate in a call rotation that is proportionate to the number of physicians in the market, ensuring work-life balance for all providers.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as required.
- Support and promote company values and uphold high standards of patient satisfaction, HEDIS compliance, and hospital admission reduction.
- Engage with community programs such as health-promoting activities for seniors, including exercise and arts, provided at no cost to patients.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Proficiency in common office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is advantageous but not required.
