Primary Care Physician (PCP) Overview
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our innovative healthcare team in Phoenix, AZ. As a Primary Care Physician, you will play a vital role in providing patient-centered, value-based care to an adult and senior patient population within a modern outpatient clinic. This full-time opportunity is ideal for a physician who is committed to delivering high-quality care in a supportive and collaborative environment.
Responsibilities
- Deliver comprehensive primary care services to a diverse adult and senior patient population, primarily in an outpatient clinic setting in Phoenix, AZ.
- Focus on managing patients with multiple chronic conditions, such as heart disease, diabetes, and hypertension, following a value-based care model.
- Maintain a manageable daily patient load (average 10-12 patients per day) to ensure personalized and attentive care.
- Build and manage a patient panel of up to 650 individuals, fostering continuity and long-term relationships with patients.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive full support from clinic leadership.
- Participate in a rotating call schedule with other physicians in the market, with call frequency dependent on the total number of providers.
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Utilize electronic health records and maintain accurate, up-to-date patient documentation.
- Promote patient engagement through health education, wellness activities, and community-focused programs offered by the clinic.
- Contribute to clinic goals related to patient satisfaction, adherence to healthcare quality measures (such as HEDIS compliance), and reduction of unnecessary hospitalizations.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Fellowship in Geriatrics is considered a plus, but not required.
- Current, unrestricted, and active medical license to practice medicine.
- Valid and current DEA registration.
- Proficiency in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Demonstrated ability to work effectively within a multidisciplinary team and communicate clearly with patients, families, and colleagues.
