Primary Care Physician (PCP) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our mission-driven, physician-led primary care organization in New Orleans, LA. As a Primary Care Physician, you will play a pivotal role in transforming healthcare for the senior population, delivering high-quality, value-based care in a supportive, collaborative environment. This full-time position offers you the chance to make a meaningful impact in the community while working in a practice that prioritizes both patient well-being and provider satisfaction.
Responsibilities
- Provide comprehensive primary care services in an outpatient setting, focusing on seniors with multiple chronic conditions.
- Manage a patient panel of up to 450 individuals, ensuring continuity and quality of care with a maximum of 18 patients per day.
- Collaborate with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes and streamline clinic operations.
- Coordinate care with in-house consulting specialists in cardiology, podiatry, and other disciplines to deliver holistic healthcare solutions.
- Utilize value-based care principles to improve patient health outcomes and contribute to the organization's mission of honoring seniors with affordable, VIP care.
- Maintain thorough and accurate clinical documentation and participate in quality improvement initiatives.
- Participate in administrative duties and dedicated administrative time scheduled within the workweek.
- Engage in interdisciplinary team activities, including holistic health services such as acupuncture, Tai Chi, yoga, and nutrition programs offered at the clinic.
- Uphold high ethical and professional standards, ensuring compliance with organizational policies and regulatory requirements.
Qualifications
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to patient-centered, value-based healthcare for diverse senior populations.
- Excellent communication and interpersonal skills to foster strong relationships with patients, families, and team members.
- Demonstrated ability to work collaboratively within a multidisciplinary team.
Clinic Environment and Benefits
- Monday through Friday schedule, with clinic hours from 7:30 AM to 5:00 PM; last patient scheduled at 4:15 PM.
