Primary Care Physician (PCP) – Cleveland, OH – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our physician-led, mission-driven organization in Cleveland, OH. As a Primary Care Physician, you will play a vital role in transforming healthcare delivery for seniors by providing comprehensive, value-based care that supports better health outcomes and honors our patients. Responsibilities
- Provide high-quality, comprehensive primary care to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Conduct patient assessments, diagnose conditions, develop treatment plans, and monitor ongoing health needs in an outpatient clinic setting.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to coordinate care and ensure excellent patient experiences.
- Utilize value-based care principles to enhance patient outcomes, emphasizing preventive health and chronic disease management.
- Maintain a schedule of approximately 18 patients per day, with administrative time included to support thorough patient care and documentation.
- Participate in partnership opportunities, with many physicians achieving partner status within 12-18 months.
- Refer patients to in-house consulting specialists as needed, including cardiology and podiatry, and support access to holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Ensure compliance with all clinical protocols, quality standards, and regulatory requirements.
- Contribute to a culture of respect, inclusivity, and excellence in patient care.
Qualifications
- MD or DO degree from an accredited medical school.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Current, unrestricted medical license to practice as a physician in Ohio.
- Demonstrated commitment to providing patient-centered, evidence-based care.
- Strong communication, teamwork, and organizational skills.
- Experience working with senior populations and managing chronic diseases is preferred.
