Primary Care Physician (PCP) OverviewWe are seeking a dedicated and compassionate Primary Care Physician (PCP) for a full-time opportunity in Clearwater, FL. Responsibilities- Provide comprehensive, patient-focused primary care services to a panel of up to 450 seniors, primarily managing patients with multiple chronic diseases.
- Conduct scheduled appointments, seeing an average of 18 patients per day, Monday through Friday, within clinic hours of 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.).
- Deliver 100% outpatient care with no required night or weekend shifts, promoting a healthy work/life balance.
- Utilize a value-based care model to improve patient outcomes and satisfaction.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, and referral specialists, to ensure a seamless patient experience.
- Participate in administrative duties with dedicated time allotted within your schedule.
- Engage with in-house consulting specialists such as cardiology and podiatry to support holistic patient care.
- Contribute to a culture focused on honoring seniors and delivering better health outcomes.
- Successful completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Strong commitment to patient-centered, value-based care, particularly for senior populations with complex medical needs.
- Excellent communication, organizational, and interpersonal skills.
