Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join a value-based healthcare network specializing in care for Medicare patient populations. This full-time position is located in Chicago, IL, where you will play a pivotal role in providing equitable and effective primary care within an innovative neighborhood clinic model. The position is ideal for board-certified or board-eligible Internal Medicine or Family Medicine physicians who are passionate about preventive medicine and patient-centered care.
Key Responsibilities
- Deliver high-quality primary care, emphasizing value-based outcomes, preventive services, and comprehensive patient management.
- Manage a patient volume of approximately 13-16 patients per day, allowing for adequate time with each patient (20 minutes for regular visits and 40 minutes for full diagnostics and new patients).
- Work collaboratively with a supportive care team, including a scribe, medical assistant, and social worker, to enhance the patient experience and provider efficiency.
- Utilize state-of-the-art clinic resources, such as onsite laboratory services with a dedicated phlebotomist and access to RubiconMD for specialist consultations.
- Document patient care accurately using electronic medical record systems, including Canopy and Greenway.
- Participate in community engagement activities, including patient wellness events, exercise classes, and educational programs, to promote holistic health in the local Medicare community.
- Contribute to a collaborative work environment with other providers, including nurse practitioners and fellow physicians, typically within a team of three to five at each clinic location.
- Support initiatives to reduce hospital admissions and improve overall patient outcomes through preventive medicine and coordinated care.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited program.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- Bilingual in Spanish is required to best serve the diverse patient population.
- Fellowship training in Geriatrics is preferred but not required.
- Residents are not eligible for this opportunity.
