Primary Care Physician (PCP) Job DescriptionPrimary Care Plus is actively seeking a dedicated and experienced Primary Care Physician (PCP) to join our expanding team in Baton Rouge, Louisiana. This full-time position offers a unique opportunity to be part of a clinically integrated network focused on delivering high-quality, coordinated healthcare to Medicare Advantage, Quality plan, and Accountable Care Organization (ACO) members. As a Primary Care Physician, you will play a pivotal role in improving patient outcomes through comprehensive care in an ambulatory health care center setting.
Responsibilities- Provide direct patient care and thorough assessments, focusing on both preventive and chronic disease management in the outpatient setting.
- Assist in developing individualized plans of care, while offering patient, family, and significant other counseling and education to promote health and wellness.
- Order and interpret appropriate laboratory and diagnostic studies to guide clinical decision-making.
- Prescribe medications and recommend evidence-based treatments as necessary for patient care.
- Refer patients to relevant specialists, such as dermatology, neurology, ophthalmology, endocrinology, surgery, intensive care, infectious disease, hematology, psychiatry, social services, and dietary services, when clinically indicated.
- Document patient care activities through comprehensive progress notes and summary reports to ensure continuity and quality of care.
- Participate in patient care rounds and clinical conferences to discuss patient management strategies and share insights with the multidisciplinary team.
- Communicate and collaborate with clinic support staff, APRNs, RN Care Managers, and the Quality Improvement team to ensure coordinated and effective patient care.
- Recognize urgent or emergent situations and initiate appropriate life-saving interventions promptly.
- Engage in risk and quality management programs, clinical meetings, and other required departmental meetings to support organizational goals and regulatory compliance.
- Medical degree (MD or DO) with completion of an accredited residency in Internal Medicine or Family Medicine.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Current, unrestricted medical license to practice as a physician in Louisiana.
- Strong clinical documentation skills with proficiency using electronic medical records (EMR) and data-driven practice management tools.
- Ability to function independently and collaboratively within a multidisciplinary care team.
- Excellent interpersonal, communication, and organizational skills.
- Commitment to quality improvement and patient-centered care.
