Primary Care Physician (PCP) Job Description

Primary Care Plus is actively seeking a dedicated Primary Care Physician (PCP), specializing in either Internal Medicine or Family Medicine, to join our expanding team in Houma, LA. As a full-time member of our clinical operations, the Primary Care Physician will play a crucial role in delivering comprehensive, high-quality healthcare to Medicare Advantage, Quality plan, and ACO patients. Our practice is part of a clinically integrated network focused on coordinated care that results in superior outcomes for our diverse patient population.

At Primary Care Plus, our collaborative clinical team includes physicians, APRNs, RN Care Managers, clinic support staff, and a Quality Improvement team. Together, we work to provide seamless care for patients in both outpatient and inpatient settings. Our robust transitional care and high-risk outreach programs equip our physicians with the necessary information to manage patient health beyond the walls of the clinic. We leverage advanced electronic medical records and data analytics to proactively identify and support high-risk patients, aiming to enhance their quality of life.

Our organization fosters a results-oriented environment where Primary Care Physicians are evaluated based on patient outcomes, not on volume or productivity metrics. Most PCPs manage patient panels in the 600-800 range and appointments are scheduled to allow for thorough, individualized attention, typically lasting 30-60 minutes per patient. Continuous feedback is offered to support clinical and business objectives, including quality measures, diagnostic accuracy, and hospitalization rates.

As a Primary Care Physician, you will provide direct patient care, conduct assessments, and participate in clinical rounds and conferences. You will be responsible for in-depth documentation through comprehensive progress notes and summaries. The role requires a blend of autonomy and teamwork, as you will function independently while collaborating closely with other healthcare professionals. Compliance with organizational guidelines, regulatory requirements, and departmental procedures is integral to this role. Participation in quality management initiatives and clinical meetings is also expected.
