Primary Care Plus is seeking a dedicated and skilled Primary Care Physician (PCP) to join our expanding clinical team in New Orleans, LA. This position is available as a full-time role within a collaborative, patient-centered environment. The Primary Care Physician (PCP) will provide direct patient care and play a key role in delivering high-quality, coordinated healthcare to Medicare Advantage, Quality plan, and Accountable Care Organization (ACO) members. The ideal candidate will be board certified or board eligible in Internal Medicine or Family Medicine.

Our team includes physicians, Advanced Practice Registered Nurses (APRNs), Registered Nurse (RN) Care Managers, clinic support staff, and a Quality Improvement team. Together, we work to support patients in both outpatient and inpatient settings, emphasizing a seamless care experience. Responsibilities
- Develop individualized plans of care for patients and provide counseling and education to patients, families, and significant others.
- Order and interpret appropriate laboratory and diagnostic studies to ensure accurate patient assessments.
- Prescribe and manage medications and treatment plans tailored to patient needs.
- Refer patients to specialists, such as dermatology, neurology, ophthalmology, endocrinology, surgery, intensive care, infectious disease, hematology, psychiatry, social services, or dietary as clinically indicated.
- Document patient encounters thoroughly using in-depth progress notes and summaries in the electronic medical record system.
- Participate in patient care rounds and interdisciplinary conferences to coordinate care strategies.
- Communicate patient management strategies to all members of the care team, ensuring alignment and continuity of care.
- Collaborate with members of the multidisciplinary team to develop and implement successful patient management strategies that meet patients' health needs.
- Recognize and respond to urgent situations requiring immediate intervention and initiate life-saving procedures as necessary.
- Engage in clinical meetings, quality management programs, and ongoing regulatory compliance activities.
Qualifications
- Medical degree (MD or DO) with completion of an accredited residency program in Internal Medicine or Family Medicine.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Unrestricted medical license to practice as a Primary Care Physician in Louisiana.
- Strong clinical skills with the ability to function independently as well as collaboratively within a multidisciplinary team.
- Excellent communication, documentation, and organizational skills.
- Commitment to providing compassionate, high-quality patient care with a focus on outcomes rather than volume.
