Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our innovative healthcare center in Birmingham, AL on a full-time basis. This is an exceptional opportunity for a physician who values patient-centered care and values-based medicine in an outpatient setting. As a member of our clinical team, you will provide comprehensive primary care services to a panel of senior patients, many of whom present with complex chronic conditions. Our new center, opening in May 2025, is focused on delivering a holistic, supportive healthcare experience while fostering genuine relationships with our patients and their families.
Key Responsibilities
- Deliver high-quality primary care to a senior patient population, primarily managing patients with multiple chronic diseases including heart disease, diabetes, and hypertension.
- Practice in an outpatient-only setting, working Monday through Friday from 8:00 a.m. to 5:00 p.m., with a call schedule determined by the number of providers in the market.
- Maintain an average daily patient census of 10-12, with a maximum panel size of 650 patients, ensuring you have adequate time for comprehensive, individualized care.
- Work collaboratively within a team-based care model, supported by a dedicated care team that includes a medical assistant, scheduler, referral specialist, and engaged leadership.
- Contribute to quality improvement efforts, focusing on patient satisfaction, adherence to HEDIS measures, and reducing hospital admissions and sick days.
- Utilize electronic medical records and demonstrate proficiency in office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Engage with patients in an environment that values wellness, respect, and community, with support services such as transportation and wellness activities available to patients at no cost.
Required Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Demonstrated proficiency with common primary care procedures and basic office-based interventions.
