Primary Care Physician (MD/DO) – Outpatient, Value-Based Care Job Overview
We are seeking an experienced and compassionate Primary Care Physician (MD/DO) to join our patient-focused healthcare team in Philadelphia, PA. This is a full-time, outpatient position that utilizes a value-based care model to deliver exceptional service to a senior patient population. Responsibilities
- Deliver comprehensive outpatient primary care to a senior patient population, focusing on those with chronic and complex medical conditions such as heart disease, diabetes, and hypertension.
- Utilize a value-based care approach, striving to improve patient outcomes, satisfaction, and overall wellness.
- Manage an average of 10-12 patients per day, with a maximum panel size of 650 patients to ensure individualized attention and continuity of care.
- Lead a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive full support from clinic leadership.
- Participate in a shared call schedule, with frequency determined by the total number of physicians in the local market (for example, 1:32 weeks with 32 providers).
- Provide office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Ensure compliance with HEDIS measures and contribute to reducing unnecessary hospitalizations among the senior population.
- Contribute to a vibrant clinic environment by supporting initiatives and activities designed to promote wellness, such as exercise and art programs for seniors.
- Model and reinforce the organization’s values in daily practice, fostering trust and respect among patients, families, and staff.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM); fellowship in Geriatrics is a plus but not required.
- Possess a current, active, and unrestricted medical license in the state of Pennsylvania.
- Maintain current DEA registration.
- Demonstrated proficiency in primary care office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Strong commitment to value-based, patient-centered care and experience working with older adults with multiple comorbidities preferred.
