We are seeking an experienced and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Philadelphia, PA. This full-time, outpatient opportunity is designed for a dedicated physician interested in value-based care, serving a senior patient population with complex medical needs. As a Primary Care Physician, you will play a pivotal role in improving patient outcomes and promoting wellness within the community.
Key Responsibilities
- Provide comprehensive primary care services to a panel of senior patients, focusing on the management of chronic and complex conditions such as heart disease, diabetes, and hypertension.
- Deliver outpatient care in a value-based care model, emphasizing preventive medicine, continuity of care, and patient satisfaction.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership.
- Maintain a manageable patient volume, averaging 10-12 patients per day, with a maximum panel size of 650 patients to ensure quality care and personalized attention.
- Participate in a shared call schedule, with frequency determined by the total number of providers in the local market.
- Utilize advanced clinical skills in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Contribute to organizational goals by meeting key performance indicators such as patient satisfaction, adherence to company values, HEDIS compliance, and reducing hospital admissions.
- Engage with patients in a supportive environment that encourages wellness through additional clinic offerings, including health-promoting activities tailored to the needs of seniors.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Fellowship training in Geriatrics is a plus, but not required.
- Current, active, and unrestricted medical license to practice medicine.
- Current and valid DEA registration.
- Proficiency in office-based procedures, including but not limited to suturing, joint injections, skin biopsies, and basic wound care.
