Primary Care Physician (MD/DO) – Outpatient, Value-Based Care Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Philadelphia, PA. This is a full-time outpatient opportunity focused on delivering high-quality, value-based care to a senior patient population with complex medical needs. As a Primary Care Physician, you'll play a pivotal role in improving patient outcomes while being fully supported by an experienced care team and leadership committed to your professional growth.
Responsibilities
- Provide comprehensive outpatient primary care to a panel of senior patients, typically managing 10-12 patients per day.
- Deliver patient-centered care utilizing a value-based model, focusing on health outcomes, patient satisfaction, and effective chronic disease management.
- Lead and collaborate with your dedicated care team, including a medical assistant, scheduler, referral specialist, and receive robust support from leadership.
- Participate in initiatives to promote wellness, continuity of care, and community engagement through activities and services offered at the clinic.
- Maintain compliance with HEDIS measures and contribute to reducing unnecessary hospitalizations and sick days among your patient panel.
- Utilize proficient procedural skills in office-based care, including suturing, joint injections, skin biopsies, and basic wound care.
- Engage in minimal call responsibilities, with call frequency dependent on the total number of providers in the market.
- Uphold the organization’s mission and values in every aspect of patient care and team collaboration.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine in the state of Pennsylvania.
- Current DEA registration.
- Fellowship training in Geriatrics is a plus, but not required.
- Experience or proficiency in office procedures such as suturing, joint injections, skin biopsies, and wound care.
- Strong commitment to practicing evidence-based medicine and delivering care in a collaborative, team-oriented setting.
