As a Primary Care Physician (MD/DO) specializing in Internal Medicine (IM) or Family Medicine (FM), you will be part of a dynamic team focused on delivering affordable, value-based VIP care for the senior population. Responsibilities
- Deliver comprehensive primary care in an outpatient setting to a dedicated senior patient panel, typically managing 18 patients per day.
- Focus on value-based care for seniors, many with complex chronic conditions (average 3-5 chronic diseases).
- Collaborate closely with a dedicated care team, including medical assistants, schedulers, and referral specialists, to ensure high-quality, coordinated patient care.
- Participate in a Monday through Friday clinic schedule, with operating hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.), providing predictable hours and no night or weekend shifts.
- Maintain a manageable patient panel size, with a maximum of 450 patients, allowing for strong provider-patient relationships and personalized care.
- Utilize built-in administrative time within your schedule for charting, care coordination, and collaboration.
- Engage with in-house consulting specialists, such as cardiology and podiatry, and leverage holistic health services including acupuncture, tai chi, yoga, and nutrition for comprehensive patient wellness.
- Facilitate patient access to care with the support of door-to-door transportation services for those in need.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, patient-centered primary care for senior populations.
