Primary Care Physician (MD/DO) – Internal Medicine or Family Medicine Overview

Primary Care Plus is seeking a highly skilled and compassionate Primary Care Physician (MD/DO) specializing in Internal Medicine or Family Medicine to join our expanding team in Algiers, Louisiana. As a vital part of our clinically integrated network, you will play a key role in providing comprehensive care to Medicare Advantage, Quality Plan, and Accountable Care Organization (ACO) members. This is a full-time opportunity within a supportive, patient-centered clinic environment.

Responsibilities

• Develop and implement individualized patient care plans to address chronic and acute medical needs.
• Provide patient and family education, counseling, and preventive care guidance to foster long-term wellness.
• Order, interpret, and follow up on laboratory and diagnostic studies to inform clinical decision-making.
• Prescribe medications, therapies, and treatments in accordance with current clinical guidelines and standards of care.
• Refer patients to appropriate specialists for advanced evaluation and management when medically necessary.
• Maintain thorough, accurate, and timely documentation of patient encounters, progress notes, and care summaries.
• Participate actively in patient care rounds, case conferences, and interdisciplinary team meetings to enhance care coordination.
• Collaborate with multidisciplinary teams—including APRNs, RN Care Managers, and Quality Improvement staff—to ensure optimal patient outcomes.

Qualifications

• Doctor of Medicine (MD), Doctor of Osteopathy (DO), or equivalent (M.B.B.S).
• Board certification or eligibility in Internal Medicine or Family Medicine.
• Active and unrestricted medical license to practice in Louisiana.
• Valid DEA license for prescribing medications.

Preferred Qualifications

• Certification in Basic Cardiac Life Support (BCLS).
• Board certification in Internal Medicine or Family Medicine.
• At least one year of clinical experience in a geriatric, adult, or family practice setting.
