Primary Care Physician (MD/DO) – Memphis, TN – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) specializing in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine to join our physician-led, mission-driven healthcare organization in Memphis, TN. Our organization focuses on providing high-quality, value-based care to the senior population, delivering comprehensive and affordable healthcare solutions designed to improve patient outcomes and support the well-being of our communities. As a Primary Care Physician, you will play a key role in upholding our mission to honor seniors with better health through VIP-level primary care.
Responsibilities
- Manage a small patient panel of up to 450 patients, focusing primarily on seniors with multiple chronic conditions.
- Conduct patient appointments in an outpatient clinic setting, averaging 18 patients per day.
- Deliver care Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m., and no nights or weekends required.
- Implement value-based care practices to achieve optimal health outcomes for each patient.
- Work collaboratively with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to ensure coordinated, efficient patient care.
- Utilize administrative time included in your regular schedule for documentation and care planning.
- Participate in partnerships within the organization, with the potential for advancement typically within 12-18 months.
- Access in-house consulting specialists such as Cardiology and Podiatry, as well as holistic health services including acupuncture, tai chi, yoga, and nutrition counseling.
- Support patient access by coordinating with transportation services available to patients at no charge.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Residency training completed in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing patient-centered, value-based primary care.
- Strong clinical, communication, and organizational skills, with the ability to work effectively as part of an interdisciplinary care team.
