Primary Care Physician (MD/DO, IM/FM, BC/BE) – Cleveland, OH – Full-time
We are seeking a dedicated and mission-driven Primary Care Physician (MD/DO, IM/FM, BC/BE) to join our growing, physician-led organization in Cleveland, OH. Our primary care model is focused on transforming healthcare for seniors with complex needs, supporting each patient with comprehensive, value-based care. Responsibilities
- Provide high-quality, evidence-based primary care to a small patient panel of up to 450 seniors, managing patients with multiple chronic diseases.
- Deliver comprehensive outpatient care, including diagnosis, treatment, and preventive services, in a 100% outpatient clinic setting.
- Work collaboratively with a dedicated support team, including your own Medical Assistant, Scheduler, and Referral Specialist, to ensure coordinated and efficient care for each patient.
- See an average of 18 patients per day, with administrative time built into your schedule for documentation and care coordination.
- Practice value-based care by focusing on quality outcomes and patient satisfaction rather than volume-based metrics.
- Participate in interdisciplinary case reviews and collaborate with in-house consulting specialists, such as Cardiology and Podiatry, to address the full spectrum of your patients’ needs.
- Engage in holistic health programs offered at the clinic, including acupuncture, tai chi, yoga, and nutrition counseling, to support patient wellness.
- Utilize clinic resources, such as door-to-door transportation for patients, to reduce barriers to care.
Qualifications
- MD or DO degree from an accredited medical school.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible (BC/BE) in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to providing patient-centered, compassionate care to a diverse senior population.
- Strong communication, teamwork, and organizational skills.
Work Hours
- Full-time position with a Monday through Friday schedule.
- Clinic hours are 7:30 a.m. to 5:00 p.m.; last patient scheduled at 4:15 p.m.
- No nights or weekends required, supporting work-life balance.
- Administrative time is included within the scheduled workday.
