Primary Care Physician (MD/DO, FM/IM) – Primary Care Job OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) – Primary Care to join our physician-led and mission-driven healthcare organization in Margate, FL. As a full-time member of our clinical team, you will play a key role in providing high-quality primary care to a panel of senior patients, focusing on delivering value-based, patient-centered healthcare. Responsibilities- Provide comprehensive outpatient primary care services to a dedicated patient panel, focusing on senior adults with multiple chronic conditions.
- Manage a small patient panel, with a maximum of 450 patients, ensuring personalized attention and continuity of care.
- Conduct patient visits (up to 18 per day), including assessment, diagnosis, treatment, and ongoing management of acute and chronic medical conditions.
- Collaborate with an interdisciplinary care team that includes a medical assistant, scheduler, and referral specialist to support patient care and administrative needs.
- Work within a Monday through Friday schedule, with clinic hours from 7:30 a.m. to 5:00 p.m. and the last patient scheduled at 4:15 p.m., ensuring work/life balance.
- Participate in value-based care initiatives and quality improvement efforts to optimize patient outcomes and organizational performance.
- Utilize administrative time included in your schedule for patient follow-up, care coordination, and documentation.
- Refer patients to in-house consulting specialists, such as cardiology and podiatry, as needed for comprehensive care management.
- Support patients’ holistic health needs through access to services like acupuncture, Tai Chi, yoga, and nutrition counseling available at the clinic.
- Ensure all care is provided in accordance with established clinical guidelines and regulatory standards.
- Medical degree (MD or DO) and completion of a residency program in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to delivering value-based, patient-centered care, particularly for senior populations with complex medical needs.
- Strong interpersonal skills and the ability to work collaboratively within a multidisciplinary team.
- Excellent organizational and communication skills to ensure effective patient management and care coordination.
