Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care, PCP) Job OverviewWe are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our innovative healthcare team in St. Louis, MO. This full-time, outpatient role is centered around delivering comprehensive, value-based care to a senior patient population in a supportive and modern clinical environment. As a Primary Care Physician, you will play a pivotal role in improving the health and wellness of your patients, working closely with a multidisciplinary team to ensure the highest standards of care.
Responsibilities- Deliver high-quality primary care services to a panel of senior patients, focusing on individuals with complex health conditions such as heart disease, diabetes, and hypertension.
- Provide care in an outpatient setting, maintaining continuity and building lasting relationships with patients and their families.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership support, to deliver coordinated care.
- Utilize a value-based care model to improve health outcomes, reduce unnecessary hospitalizations, and promote overall wellness.
- Participate in performance-based bonus programs, with metrics including patient satisfaction, adherence to company values, HEDIS compliance, and reduction in hospital sick days.
- Practice evidence-based medicine and perform minor office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Maintain a manageable patient volume, averaging 10-12 patient visits per day, with a maximum panel size of 650.
- Adhere to a Monday through Friday schedule (8:00 a.m. – 5:00 p.m.), with a call schedule determined by the number of physicians in the local market.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted license to practice medicine in the state of Missouri.
- Current DEA registration required.
- Fellowship training in Geriatrics is a plus but not required.
- Proficient in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
