Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) for a full-time role in St. Petersburg, FL. Responsibilities
- Provide comprehensive outpatient primary care to a panel of senior patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage a manageable daily patient load (average 10-12 patients per day) to ensure high-quality, individualized care.
- Work within a value-based care model, emphasizing preventive care, patient satisfaction, and reduction in hospitalizations.
- Lead and collaborate with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership support.
- Participate in quality improvement initiatives, including HEDIS compliance, and contribute to the achievement of organizational values and patient satisfaction goals.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Maintain accurate electronic health records and documentation in compliance with medical standards and regulations.
- Participate in a shared call schedule based on the number of physicians in the market (e.g., 1:32 weeks if 32 providers).
- Engage in ongoing communication with patients, families, and the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Active, unrestricted medical license to practice in the state of Florida.
- Current DEA registration required.
- Proficiency in office-based procedures including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus but not required.
- Strong interpersonal, organizational, and communication skills, with a commitment to patient-centered care.
- Comfort working with seniors with multiple chronic conditions.
