Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) Overview
We are seeking a dedicated and compassionate Primary Care Physician to join our innovative healthcare team in Bradenton, FL. This is a full-time, outpatient opportunity with a focus on value-based care, serving a senior patient population. Responsibilities
- Provide comprehensive primary care to an assigned panel of adult and senior patients, focusing on patients with multiple, complex conditions such as heart disease, diabetes, and hypertension.
- Deliver outpatient care following a value-based care model, prioritizing both quality and efficiency in treatment plans.
- Work collaboratively within a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership, to ensure coordinated and effective patient care.
- Maintain patient panel size, with a maximum of 650 patients and an average daily volume of 10-12 patients.
- Utilize evidence-based clinical practices, focusing on HEDIS measures, patient satisfaction, and reduction of avoidable hospitalizations.
- Participate in call schedule as required, with frequency based on the number of providers in the market (e.g., 1:32 weeks if 32 physicians are present).
- Perform common office procedures, including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Uphold the organization's values by fostering respectful, patient-centered interactions and advocating for wellness at every opportunity.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) (required).
- Fellowship training in Geriatrics is a plus but not required.
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficiency in common office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Strong commitment to patient-centered care and experience working with senior populations with multiple chronic conditions is preferred.
