Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) Overview

As a primary care physician, you will play a vital role in supporting our mission to enhance the quality of life for our patients, particularly seniors with complex health needs, while working within a value-based care model. Responsibilities
- Provide comprehensive outpatient primary care services to a panel of senior patients with multiple chronic conditions, such as heart disease, diabetes, and hypertension.
- Utilize a value-based care approach to deliver patient-centered care, focusing on improving patient satisfaction and health outcomes.
- Maintain an average daily patient volume of 10–12, ensuring each patient receives individualized attention and care.
- Work collaboratively as part of a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive full support from clinic leadership.
- Participate in a call schedule determined by the number of providers in the market, ensuring shared responsibilities among physicians.
- Engage in activities that support patient wellness, including promoting participation in exercise, art, and other health-promoting programs offered by the clinic.
- Practice within a newly opened center, contributing to the growth and development of the clinic and its community presence.
- Support continuity of care by building long-term relationships with patients and their families.
- Monitor and report on key performance indicators such as HEDIS compliance, patient satisfaction, and efforts to reduce hospitalizations.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus but not required.
- Current, unrestricted, and active license to practice medicine in the state of Ohio.
- Current DEA registration.
- Proficiency in office-based procedures, such as suturing, joint injections, skin biopsies, and basic wound care.
