Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) About the OpportunityThis is a full-time Primary Care Physician position based in Jacksonville, FL, offering an exciting opportunity to join a new and innovative healthcare provider committed to improving the lives of patients through a value-based care model. Key Responsibilities- Provide comprehensive outpatient primary care services to a senior patient population, typically managing patients with multiple complex chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care using a value-based model, focusing on patient health outcomes, satisfaction, and overall wellness.
- Work collaboratively as part of a multi-disciplinary team, including medical assistants, schedulers, referral specialists, and supportive leadership.
- Manage an average of 10-12 patients per day within a maximum panel size of 650, ensuring personalized attention and continuity of care.
- Participate in a Monday through Friday schedule, 8:00 a.m. to 5:00 p.m., with shared call responsibilities determined by the number of physicians in the market (for example, 1:32 weeks for 32 providers).
- Utilize strong clinical skills in office procedures, including suturing, joint injections, skin biopsies, and basic wound care, as needed.
- Engage in regular performance reviews, with bonuses based on patient satisfaction, adherence to company values, HEDIS compliance, and reducing hospitalizations.
- Contribute to the supportive, patient-centered culture of the clinic, helping to ensure that patients have access to additional resources such as transportation, exercise, art, and wellness activities.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted license to practice medicine in Florida.
- Active DEA registration.
- Experience in geriatrics or a fellowship in geriatrics is a plus but not required.
- Proficient in common office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
