Primary Care Physician (MD/DO, FM/IM, Outpatient, PCP) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM, Outpatient, PCP) for a full-time opportunity in Philadelphia, PA. This role is ideal for a board-certified or board-eligible physician in Family Medicine, Internal Medicine, or Geriatric Medicine who is committed to providing high-quality, value-based care to a senior patient population. Key Responsibilities
- Deliver comprehensive primary care to adult and senior patients, focusing on those with multiple chronic conditions.
- Manage a small patient panel (up to 450 patients) to provide personalized, attentive care.
- Conduct patient visits, averaging 18 per day, during clinic hours Monday through Friday (7:30 a.m. to 5:00 p.m., with last patient scheduled at 4:15 p.m.).
- Engage in value-based healthcare delivery, emphasizing quality outcomes and patient satisfaction.
- Collaborate with a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist to ensure coordinated patient care.
- Utilize in-house consulting specialists such as Cardiology and Podiatry, as well as holistic health services including Acupuncture, Tai Chi, Yoga, and Nutrition.
- Participate in administrative activities and maintain accurate patient records during scheduled administrative time.
- Provide 100% outpatient, clinic-based care with no nights or weekends required.

Qualifications
- MD or DO degree from an accredited medical school.
- Residency training in Family Medicine (FM), Internal Medicine (IM), or Geriatrics.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Commitment to value-based, patient-centered care.
- Spanish language proficiency is preferred but not required.
- Strong interpersonal, communication, and organizational skills.
