Primary Care Physician (MD/DO, FM/IM) - Outpatient Overview
We are seeking a dedicated and mission-driven Primary Care Physician (MD/DO, FM/IM) for a full-time position in St. Louis, MO. As part of a physician-led organization focused on providing comprehensive outpatient care, you will play a vital role in delivering high-quality healthcare to a senior patient population. Responsibilities
- Provide outpatient primary care services to a panel of up to 450 senior patients, typically managing 3-5 chronic conditions each.
- Conduct patient visits during clinic hours: Monday through Friday, 7:30 a.m. to 5:00 p.m., with the last appointment scheduled at 4:15 p.m.
- Manage an average daily schedule of 18 patients, allowing for meaningful patient interactions and continuity of care.
- Participate in a value-based care model focused on patient outcomes and preventive health.
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to streamline clinical operations and enhance patient support.
- Utilize administrative time built into the schedule to complete documentation and coordinate patient care efficiently.
- Coordinate patient care with in-house consulting specialists, such as Cardiology and Podiatry, to provide holistic support.
- Leverage holistic health services available at the clinic, including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Ensure compliance with all clinical protocols, professional standards, and organizational policies.
- Support the organization's mission to deliver accessible, high-quality care to underserved populations, especially seniors.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Residency training in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Experience and passion for working with senior populations and managing chronic diseases are highly valued.
- Commitment to delivering patient-centered care within a value-based model.
