Primary Care Physician (MD/DO, FM/IM, Outpatient) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM, Outpatient) for a full-time opportunity in Phoenix, AZ. This role is ideal for a medical professional passionate about providing comprehensive, patient-centered healthcare to a diverse senior population in a modern, value-based outpatient clinic setting. As a Primary Care Physician, you will play a key role in improving the health and well-being of patients while working alongside a supportive multidisciplinary care team.
Responsibilities
- Provide high-quality, outpatient primary care to adult and senior patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage a panel of up to 650 patients, averaging 10-12 patient visits per day, ensuring continuity of care and patient satisfaction.
- Collaborate with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership support, to deliver coordinated and efficient care.
- Participate in a value-based care model that rewards quality, patient satisfaction, and clinical outcomes, including HEDIS compliance and reduction of hospital admissions.
- Utilize clinical skills such as suturing, joint injections, skin biopsies, and basic wound care as needed during office visits.
- Adhere to a Monday through Friday, 8:00 a.m. to 5:00 p.m. schedule, with call responsibilities distributed equitably among providers in the market.
- Engage with patients as part of a vibrant, wellness-focused community, supporting holistic health initiatives and participating in activities designed to promote senior wellness.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active medical license to practice medicine.
- Valid DEA registration.
- Proficiency in common office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
