Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time outpatient role focused on delivering value-based healthcare to senior patients. This opportunity is based in Phoenix/Mesa E., AZ, where you will join a new and innovative healthcare provider committed to improving patient outcomes through continuity of care and a wellness-centered approach. As a Primary Care Physician (MD/DO), you will play a pivotal role in fostering a vibrant, community-focused environment that prioritizes patient respect, engagement, and holistic health.
Key Responsibilities
- Deliver comprehensive outpatient primary care to a panel of senior patients, typically managing 3-5 chronic or complex diseases such as heart disease, diabetes, and hypertension.
- Work within a value-based care model, emphasizing patient satisfaction, HEDIS compliance, and reduction of hospital sick days.
- Average patient volume is 10-12 per day, supporting a focus on quality over quantity.
- Maintain a patient panel size up to 650, ensuring personalized and attentive medical care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and strong leadership support.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Participate in a rotating call schedule based on the number of providers in the region, ensuring a balanced work-life dynamic.
- Uphold and model company values in all patient and team interactions.
- Actively contribute to an inclusive, respectful, and mission-driven clinic culture.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Current, unrestricted, active medical license to practice in the state of Arizona.
- Current DEA registration required.
- Proficiency in common office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship in Geriatrics is a plus, but not required.
