Primary Care Physician (MD/DO, FM/IM, Geriatrics) – Bradenton, FL – Full-time
Our physician-led, mission-driven healthcare organization is seeking a dedicated Primary Care Physician (MD/DO) to join our team in Bradenton, FL. We are committed to delivering comprehensive, value-based healthcare to the senior community, focusing on patients with chronic conditions through a personalized and holistic approach. Responsibilities
- Provide high-quality, compassionate, and evidence-based outpatient care to a small panel of senior patients, with a maximum of 450 patients, focusing on those with multiple chronic illnesses.
- Manage an average of 18 patients per day within our clinic, supported by a dedicated care and support team, including a medical assistant, scheduler, and referral specialist.
- Utilize value-based care principles to achieve superior healthcare outcomes and patient satisfaction.
- Collaborate with in-house consulting specialists such as Cardiology and Podiatry to coordinate comprehensive patient care plans.
- Participate in holistic health initiatives provided by the clinic, including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Leverage administrative time built into your schedule to ensure thorough patient documentation and care coordination.
- Follow a Monday through Friday work schedule, with clinic hours from 7:30 AM to 5:00 PM and no nights or weekend shifts.
- Contribute to the organization’s mission by delivering affordable VIP care and honoring the health needs of the senior population in Bradenton, FL.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Demonstrated commitment to value-based care and improving outcomes for seniors with chronic illnesses.
- Strong interpersonal communication skills and the ability to work collaboratively within a multidisciplinary team.
