Primary Care Physician (MD/DO, FM/IM) - Summary- Provide comprehensive outpatient primary care services for a senior patient population with multiple complex conditions.
- Practice in a value-based care model designed to optimize patient outcomes and satisfaction.
- Be an integral part of a supportive care team, including medical assistants, schedulers, referral specialists, and leadership.
- Enjoy a structured Monday through Friday schedule with limited call requirements, promoting a healthy work-life balance.

- Deliver high-quality, evidence-based primary care to a panel of up to 650 senior patients, averaging 10-12 patient visits per day.
- Manage and treat chronic conditions such as heart disease, diabetes, hypertension, and others common to a complex senior population.
- Participate in a value-based care environment with compensation linked to patient satisfaction, adherence to company values, HEDIS compliance, and reduction of hospitalizations.
- Collaborate with a dedicated care team to ensure seamless patient care and efficient coordination of services, including referrals and follow-ups.
- Utilize office-based procedures including basic wound care, suturing, joint injections, and skin biopsies as clinically indicated.
- Maintain accurate and up-to-date medical records in compliance with regulatory and organizational standards.
- Engage in quality improvement initiatives and contribute to the ongoing development of best practices within the clinic.
- Participate in a shared call schedule, with frequency determined by the number of physicians in the market.
- Support the company mission of fostering a vibrant, wellness-focused community for patients and their families.
- Competitive base salary starting at $260,000 with total earning potential exceeding $320,000, including performance bonuses up to 20% of base salary.

- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) is mandatory.
- Current, unrestricted, active license to practice medicine in the relevant jurisdiction.
- Current and valid DEA registration for prescribing controlled substances.
- Proficiency in office-based clinical procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
- Commitment to delivering patient-centered care and working collaboratively in a team setting.
- Experienced in managing complex, chronic conditions within a senior patient population.
- Strong communicator with a compassionate approach to patient interactions.
