Primary Care Physician (MD/DO, FM/IM) - Summary- Deliver comprehensive outpatient primary care in a value-based care setting.
- Focus on seniors with complex medical conditions, ensuring personalized, holistic treatment plans.
- Work as part of a collaborative, interdisciplinary team dedicated to patient wellness and satisfaction.
- Participate in a patient-centered culture that emphasizes respect, empathy, and inclusivity.
- Take advantage of well-equipped clinics and strong administrative support to enhance patient outcomes.
- Provide high-quality, evidence-based primary care to a panel of up to 650 patients, focusing on seniors with multiple chronic conditions.
- Average 10-12 patient visits per day, allowing for thorough and attentive care.
- Utilize a value-based care model to improve patient outcomes and satisfaction.
- Participate in wellness and preventive health programs, including exercise and art activities for patients.
- Collaborate with a dedicated care team including medical assistants, schedulers, referral specialists, and leadership support.
- Engage in quality improvement initiatives, including HEDIS compliance and reduction of hospital admissions.
- Assist with office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Maintain accurate and timely medical records in compliance with regulatory guidelines.
- Participate in a rotating call schedule, designed to minimize after-hours responsibilities.

- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and wound care.
