As a Primary Care Physician (MD/DO, FM/IM), you will play a pivotal role in delivering comprehensive outpatient care within a value-based model, focusing on both preventive and chronic disease management for a senior patient population. Primary Care Physician (MD/DO, FM/IM) - Summary- Deliver outpatient primary care services to a senior patient population with complex medical needs.
- Practice within a value-based care model emphasizing quality outcomes, patient satisfaction, and continuity of care.
- Work collaboratively with a dedicated care team to optimize patient experiences and health outcomes.
- Contribute to a vibrant clinic environment that prioritizes patient wellness and community engagement.
- Provide comprehensive primary care to an assigned patient panel, focusing on seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Conduct patient assessments, develop treatment plans, and manage ongoing care in accordance with evidence-based guidelines.
- Maintain a patient panel of up to 650 individuals, averaging 10-12 patient visits per day to ensure quality and personalized care.
- Lead a multidisciplinary care team (including MA, scheduler, referral specialist) to coordinate care and facilitate smooth patient experiences.
- Participate in quality improvement initiatives, including HEDIS compliance and reduction of hospital admissions.
- Support patient engagement through clinic-offered wellness activities such as exercise and art classes.
- Utilize strong PC skills for office-based procedures including suturing, joint injections, skin biopsies, and basic wound care.
- Follow a Monday through Friday, 8am to 5pm shift schedule; participate in a shared call rotation based on the number of providers.

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Fellowship training in Geriatrics is advantageous but not required.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
