Primary Care Physician (MD/DO, FM/IM) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) for a locum tenens, full-time opportunity in Phoenix, AZ. This role is focused on providing high-quality, value-based outpatient care to a senior patient population within an innovative, patient-centered healthcare setting. Responsibilities
- Deliver comprehensive primary care services in an outpatient clinic environment, emphasizing a value-based care model.
- Manage a patient panel primarily composed of seniors, with an average of 3–5 complex chronic conditions such as heart disease, diabetes, and hypertension.
- See an average of 10–12 patients per day, maintaining a manageable and patient-focused schedule to ensure continuity and quality of care.
- Utilize a multidisciplinary care team, including medical assistants, schedulers, referral specialists, and supportive leadership, to provide holistic patient care.
- Engage patients in wellness and preventive health initiatives, including facilitating access to exercise, art, and other community-based activities at no cost to the patient.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care, utilizing proficient PC skills.
- Adhere to clinic protocols and maintain accurate, timely documentation in the electronic health record system.
- Collaborate with other healthcare providers to ensure optimal patient outcomes and continuity of care.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certification in Family Medicine (FM) or Internal Medicine (IM) required.
- Active, unrestricted medical license to practice medicine in Arizona.
- Current DEA registration required.
- Experience with office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
- Comfort with electronic health records and basic computer proficiency.
