Primary Care Physician (MD/DO, FM/IM) – Birmingham, AL – Full-timeWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join an innovative healthcare provider in Birmingham, AL. As a primary care leader in a value-based care model, you will play a central role in improving the health and wellness of a senior patient population with complex medical needs. This full-time opportunity is an excellent fit for physicians who are committed to delivering patient-centered, high-quality outpatient care in a supportive and collaborative environment.
Responsibilities- Provide comprehensive, outpatient primary care services to senior patients, focusing on individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care in alignment with value-based care principles, emphasizing patient satisfaction, clinical outcomes, and reduced hospitalizations.
- Manage a patient panel of up to 650 individuals, with an average of 10-12 patient visits per day.
- Work closely with a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive robust support from leadership.
- Participate in a shared call schedule, with call frequency determined by the number of providers in the market (e.g., 1 in 32 weeks if 32 physicians are present).
- Utilize proficient procedural skills for office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Embody the organization’s core values, contributing to a positive, patient-focused clinical culture.
- Support and promote wellness activities and community engagement programs offered at the clinic, such as exercise and art activities tailored for seniors.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license to practice medicine.
- Current DEA registration.
- Proficiency in office-based procedures relevant to primary care.
- Fellowship training in Geriatrics is a plus, but not required.
